Our MHS Kern Homeless Outreach program provides access to intensified services in areas that impede individuals’ ability to access or maintain housing. Services include behavioral health services for both mental health and substance abuse, medical health services, family and other support systems to include whole person care services for complex conditions that are interconnected.

Program Services

Our Kern Homeless Outreach delivers all services with clients as full partners in their treatment and care; these services include:

  • Access to behavioral health services
  • Trauma screening and trauma informed services
  • Housing wrap-around case management
  • Linkage to treatment
  • Increase community awareness of behavioral health and homeless services
  • Provide long term stability of individuals served

Eligibility Requirements

MHS Kern Homeless Outreach serves individuals who require more intensified case management services than traditional outpatient mental health services offer. Eligibility for services must meet the following requirements:

  • Over the age of 18.
  • Have serious and persistent mental illness.
  • High utilization of acute mental health services (e.g., P.E.C. or Psychiatric hospitalizations).
  • AB109 participants.

Referrals

Referrals are made through various resources including hospitals, jails and Kern Linkage Program, Access Center, Flood Bakersfield Ministries, Probation and other outside agencies. All referrals go through Adult Mental Health System of Care.
