Families Forward Wraparound provides mental health and supportive services with a team of caring peer coaches. Our program helps children and youth remain in the home with their families and assists them in becoming more successful in other key areas of their lives. Our wraparound process honors the family’s voices and brings people together from different parts of the family’s life, in order to provide a network of support. Families Forward Wraparound services are creative, culturally inclusive, flexible and action-oriented in our treatment approach. All services are community and home-based, in order to increase the success of treatment.

  • Substance abuse treatment and support including linkage to SUD services

  • Treatment, skills development, and psychoeducation groups

Have one of the following needs:

  • At imminent risk of higher level of care, including residential placement, incarceration, or hospitalization
  • Recent crisis stabilization at emergency screening unity or inpatient psychiatric hospitalization
  • Multiple failed placements
  • Transitioning from a higher level of care

 

Eligibility Requirements

At Families Forward, we serve children and youth who are either at risk for out of home placement or in need of support transitioning home. Our clients meet the following criteria:

  • Children, Adolescents, and Transitional Age Youth (ages 6-21) who are residents of San Diego County.
  • Have a mental health diagnosis and have full scope Medi-Cal.
  • Are at imminent risk of out of home placement (hospitalization, incarceration, or residential placement), or transitioning from out of home placement.
  • Youth must have had prior or current outpatient or inpatient therapy services, which has been insufficient to maintain stable home placement.
  • For our families that do not meet eligibility criteria, our “No Wrong Door” policy provides up to ten hours of linkage to community resources. We also provide follow up to ensure the family’s needs are met with the appropriate resources.

 

How to make a referral

1) For children with full scope Medi-Cal, please complete an online referral form, or fax/mail a referral form to the program.

2) For children without full scope Medi-Cal, you may inquire with our program and /or access the information below for further support and linkage to the right service. Families Forward is happy to help navigate you to the right treatment provider, with our “No Wrong Door” policy.
