Our Center Star Assertive Community Treatment (ACT) program offers 24-hour community-based treatment for individuals with a criminal justice background who have been diagnosed with a severe and persistent mental illness.

All clients of Center Star ACT are eligible to receive medication management and monitoring, therapy, drug and alcohol counseling, case management, vocational rehabilitation, peer counseling and support, and housing services.

Program Services

Center Star ACT provides the following services:

  • Psychiatric assessment
  • 24-hour crisis intervention and support
  • Medication management and monitoring
  • Individual and group rehabilitation counseling
  • Case management services
  • Dual recovery treatment
  • Physical health assistance
  • Supportive housing
  • Assistance with daily living activities
  • Vocational rehabilitation
  • Educational and employment development
  • Peer counseling and support

Eligibility Requirements

Center Star ACT serves individuals who need services that are more intensive than those provided by traditional outpatient mental health services and who meet the following requirements:

  • Homelessness or at high risk of homelessness
  • Reside anywhere in San Diego County
  • Medical Necessity must be established (face to face) to determine the presence of a severe psychiatric disability and need for Assertive Community Treatment
  • Level of acuity and need for intensive ACT services per LOCUS assessment (Level 4 – Medically Monitored Non-Residential Services)
  • Between the ages of 25-59
  • Have justice involvement within the past 365 days
  • Minimum one hospitalization in the past year, OR multiple ER utilizations, PERT interventions, jail mental health service and/or long-term care hospitalization.
  • Have major impairments in life functioning
  • Person is not connected to outpatient treatment
  • Person is experiencing an acute psychiatric episode that might require ACT level services
  • Is at high risk of admission to an inpatient mental health facility
  • Has a substantial need for supportive services (including care coordination and outreach mental health services) to maintain current level of functioning in the community, as evidenced by missed appointments, medication non-adherence, or inability to coordinate services from multiple agencies;
  • Does not have a case manager from another program who is able to address mental health needs

Referrals

Referrals are made through various resources, including hospitals, jails and outside agencies.
