Description: 

Our North County Transition Age Youth (TAY) program is a case management wraparound program designed to provide mental health and rehabilitation services specifically for young adults ages 18-24, diagnosed with serious mental illness. Our TAY program helps youth to achieve independence, self-sufficiency, and builds upon their individual strengths. The program assists youth with educational goals, seeking employment, volunteer opportunities, independent living, obtaining their driver’s license or other personal records, socialization activities and involvement in recreational activities

Our on-site psychiatrists and nurses assist young adult patients with chronic mental illness through medication management. Our mental health services further assist patients through the use of brief time-limited individual therapy, psycho-educational groups and support groups. Our program is funded by San Diego County Health and Human Services, Mental Health Division. Services are reimbursed through Medi-Cal and Short Doyle, a Sliding Scale formula. Our clinic does not accept private insurance.
